Once you book an appointment, you’ll receive a link via email or text to complete the new patient forms at home. Filling these out ahead of time helps us get to know your child’s needs and makes check-in quick and easy when you arrive.

Our office is in-network with many PPO insurance plans. If you have questions about dental insurance, please give us a call, and our friendly team will be happy to help verify your benefits. While our credentialing process is still underway, we can also assist with using out-of-network benefits and submitting claims.
